Informed Consent Form for Veneer Dental Treatment

To: All Health Professionals & Health Facilities within the jurisdiction of Dubai Heath Authority –Dubai 

DHA extends its thankfulness and appreciation for the relentless cooperation and support provided by its strategic partners in health sector; in an effort to provide integrated health care system and improve the levels and quality of services provided in Emirate of Dubai.

With reference to the above subject, HRS re-affirms the importance of full commitment required to abide by all below legislations,   And in the line with the initial circular for the "Dubai Dental Clinical Protocols", Ref:  (DHA/OUT/2021/0002347), Dated: 23/06/2021. Therefore, all  Health Professionals & Health Facilities within the jurisdiction of DHA, should adhere to the following:

1.    It is obligatory to use the Consent Form for Veneer Services attached to the Guidelines from 01 January 2022. Accordingly, all health professionals and health facilities within the jurisdiction of DHA must familiarize with this circular and strictly adhere to the provisions contained therein in order to avoid any legal accountability.
